Job Description
The Discharge Planner/Case Manager at Springbrook Hospital is responsible for overseeing discharge planning for patients transitioning from treatment. Key responsibilities include developing discharge plans, assessing emotional and financial concerns, coordinating with families and community resources, educating patients and families, and collaborating in multidisciplinary treatment reviews.
Qualifications: Associate’s degree in Human Services or Social Work; Bachelor’s preferred.
