Job Description

We are hiring a Medical Field Case Manager to coordinate and manage patient care outside the hospital. You will assess needs, create and implement individualized care plans, coordinate with physicians and community resources, arrange authorizations and services, monitor outcomes, and maintain timely EMR documentation. Frequent field visits and strong problem-solving are required.

Qualifications: RN or licensed social worker preferred, 2+ years case management experience, excellent communication and organizational skills, EMR proficiency, valid driver’s license, and ability to travel. Bilingual candidates are a plus.
