Job Description
Provide client-centered targeted case management to Mental Health Court clients who meet the criteria for services at Okeechobee County.
- Provide TCM services to clients from the Mental Health Court (MHC) grant.
- Utilize SEFBHN coordination to promote communication across MHC client providers.
- Implement wraparound case management methodologies.
- Attend weekly care coordination meetings with the MHC Care Coordination Team.
- Facilitate SOAR application with MHC of Okeechobee.
- Link clients to community resources.
Full-time / Salaried Position
- Minimum of a Bachelor’s Degree (required)
- CBHCM Certification required; must obtain within 6 months.
