Job Description
California Post-Acute Care is seeking a dedicated Social Services Assistant to support our Social Services program. In this role, you will assist residents with their daily psychosocial needs through room visits and interviews. You will attend family counseling meetings, facilitate resident interviews, maintain accurate records, and provide information on available benefits. Your contributions will help enhance the quality of life for residents by collaborating with staff to implement effective communication strategies. Additionally, you will oversee group activities and ensure that residents’ concerns are documented and addressed properly.
